Welcome to the Pharmacy Benefits Management (PBM) Clinical Pharmacy Practice Office (CPPO)
Since the 1970s, Clinical Pharmacists and Clinical Pharmacist Practitioners (CPP) have demonstrated their impact on improving medication safety, quality of care and clinical outcomes for Veterans as an advanced practice provider. PBM CPPO’s vision is to advance VA clinical pharmacy practice to provide the highest standard of care for our Veterans and position the VA as a model of clinical pharmacy practice excellence for diffusion and innovation throughout the U.S. PBM CPPO is committed to optimizing VA clinical pharmacy practice to improve access, promote health equity, and provide quality care in a cost-effective manner highlighting the efforts of clinical pharmacy as part of VA’s Whole Health model of care that centers Veteran experience as a top priority.
The CPP is a core member of the VA healthcare team focused on improving access and quality through the provision of comprehensive medication management (CMM). The CPP is a clinical pharmacist with a scope of practice (i.e., VA collaborative practice agreement) authorized by the medical staff. As the medication expert on the team, the CPP provides direct patient care and functions at the highest level of clinical practice, working collaboratively with autonomy and independent decision-making within the parameters of their scope of practice. VA CPPs continue to innovate and show their dedication to the VA mission of caring for Veterans through integration into interprofessional teams, addressing VA facility patient gaps in care, and providing CMM services across the continuum of care. As part of a high reliability organization, PBM CPPO develops strategies to empower the pharmacy workforce to adapt to changing needs of our Veterans ensuring all have access to CMM services provided by CPPs.
